Social workers practicing in rural and underserved communities often see the impact that rising levels of chronic disease have on individuals and families. The rurality component of health disparities must be addressed in order to affect the rate of health disparities in these areas. This webinar will discuss the rural implications of health care and offer a model of community-level practice available for social workers engaged in the health systems of rural and underserved communities.
