Sebelius settlement deemed inappropriate use of an “improvement standard” for Medicare coverage of occupational therapy, physical therapy, speech–language pathology, and skilled nursing services. Four years after the settlement was approved, however, Medicare beneficiaries continue to face challenges in accessing these services.
